- Graduate Special Registration Request Form- This form is necessary if you would like to be enrolled in a closed course, but have received the professor's approval; if you would like to adjust hours in a variable credit course; or if you would like to late add/late drop, or change sections of a course. Submit completed form here.
- MBA Audit Form- This form is required if you would like to audit a course. Proof of approval from the professor of the course is required.
- Independent Study Proposal- Please include the following form along with your application to pursue and Independent Study Course Project.
- External Class Approval Form- This form is necessary if you wish to take any courses outside of the JGSB.
- Biographical/Demographical Update Form- If you need to update or change information such as your name, social security number, citizenship status, date of birth, tax ID number, or race/ethnicity that we currently have on file for you, you must submit the form via the secure upload form (linked above,) along with a copy of one of the following: social security card, passport, birth certificate, marriage license, divorce decree, tax ID number letter, or court order providing proof of the change to be made.
